FOR NEW PATIENTS
In preparation for your New Patient appointment at Fort Worth Colon and Rectal Surgical Associates, please download the attached forms and bring the completed paperwork with you. Additionally, you will need to bring:
- Insurance card(s)
- Valid identification
- List of current prescription medication including dosage amounts
- List of vitamins, supplements, and over-the-counter medications in this list, including dosage amounts.
Co-pays and deductibles will be collected at the time of service. Additionally, payment in full is will be collected in advance for any surgical procedure or colonoscopy.
About your visit with the doctor
Be prepared to discuss your symptoms with your doctor. Having the right answers will help your doctor to better evaluate your symptoms. What is your main symptom or concern?- How long have you had it?
- How often does it occur?
- What makes it better or worse (Ex: activity, stress, medication use)?
- Is it associated with other symptoms?
- Have other people in your family experienced similar health problems?
- Has the problem been addressed by another physician or medical center?
If you have made your New Patient appointment, please download a patient registration form, and select your physician to download the appropriate new patient form.
